FORMS: Parents, please complete the school information forms in preparation for the new school year. Each form provides parents with two options for completion. Parents can either word process the forms by choosing the Microsoft Word/Excel option or hand write the forms by selecting and printing the PDF option. Parents who do not have access to technology can call the school to pick up the necessary forms. Please complete each form for each of your children and return all forms to your child’s teacher on September 7th. Be sure that all forms are legibly completed. Thank you
